Transcript Requests
Please read carefully.
Type directly onto the Official Transcript Request form, then print the completed form after you have typed your information, sign it, and send it back to the Registrar's Office via fax, mail, in person, or e-mail as a signed/scanned PDF.
Enrollment or Degree Verifications
Print the Degree/Enrollment Verification form, fill it out and sign the form, and fax or mail the form back to the Registrar's Office.
