Transcript Requests

Please read carefully.

Type directly onto the Official Transcript Request form, then print the completed form, sign it, and send it back to the Registrar's Office via fax, mail, in person, or email as a signed/scanned PDF.

All requests must be printed and physically signed (student/alumni signatures required) and returned back to the Registrar's Office via fax, mail, in person, or emailed as a signed/scanned PDF. Please note that we do not accept electronic or "mouse" signatures/stamps, and we do not accept links to documents.

Enrollment or Degree Verifications

Print the Degree/Enrollment Verification form, fill it out and sign the form, and fax or mail the form back to the Registrar's Office.
