Data Report Request Form

Faculty, Staff, and Administrators only

Be as specific as possible in your request; elaborate as necessary in the "Additional Instructions" field. Allow a minimum of five business days for your request to be filled. Please note that data for mailings can be requested with this form; however, the Registrar's Office can no longer provide printed labels. Mail merge instructions for the printing of labels can be found here.

Requests for University-wide data regarding student enrollment, data for survey purposes or data to be reported externally should be directed to the Office of Institutional Effectiveness.

Requests for University-wide data, official institutional data, and data for internal or external surveys should be directed to the Office of Institutional Effectiveness. These requests, as well as data for program assessment, external agency reporting, or news media/publications, should be directed to the Office of Institutional Effectiveness using the IFX Form.

What data fields do you need?

Please note that fields such as GPA, citizenship, race, and gender are considered confidential. Please be sure to elaborate on the need for such data elements. Additionally, students' first name in use will be provided in the report unless there is a specific need to receive students' legal first names. If legal first names are needed please indicate this as a needed data field and be sure to enter the purpose. If requesting address please specify the address type. If you have received a report of the same type previously, indicate that in the additional information box at the bottom of this page.
